To receive an estimate, you will need the following information when you call:

The name of the procedure or test
The CPT code for the procedure or test, which can be obtained from your ordering provider
Patient’s first and last name
Date of Birth
Telephone number including area code
The name of your insurance coverage and your member or policy ID, which can be found on your insurance card. Or, you can let us know if you do not have insurance or have another type of coverage
