You can request access to view records of others in MyChart

Four Types of Proxy Access

You can request or grant access as a:

  • Parent of a minor child
  • Legal guardian of a minor child
  • Legal guardian or personal representative of an adult
  • Adult granting access to another adult

With MyChart, you can request or grant access to view records of your children, spouse, parent or another adult. This is called proxy access.

How to Request Proxy Access

  • Select the access you are requesting (below) and complete the form
  • Send completed form, and any required documentation, to Bryan Health:

    • You can also drop the form off at your doctor's office or the Health Information Management office at Bryan Health hospitals
  • You can expect access within 24-48 business hours after the time we receive your form and any required documentation
    • If you have a MyChart account, you will see this person in a new 'circle' at the top of your MyChart account
    • If you don't have a MyChart account, you will receive an email with information on how to open/activate your account
      • If you did not provide an email address, you will receive a letter with the information you need to set up your MyChart account

Proxy Access Options and Forms

Parent of a Minor Child

Request for proxy access as a parent of a minor child

Form - Parent Request for Proxy Access of a Minor

For a child who is newborn-12 years old:

  • You will be able to view all records, test results and other information, and perform all functions (i.e., schedule appointments) just as a patient can in MyChart
  • This access will become limited automatically when the child turns 13 based on Nebraska law

For a child who is 13-18 years old:

  • The information you can view is more limited due to Nebraska law. You can view items such as vitals (height, weight), immunizations, allergies, family history, etc. You can also communicate with the provider to schedule appointments, request referrals, pay bills, update demographics, etc. You will not be able to view items such as: health issues, medications, test results, appointment summaries and prescription refill requests.
  • This access will automatically end when the teen turns 19 based on Nebraska law

Legal Guardian of a Minor Child

Form - Legal Guardian Requesting Proxy Access of a Minor

Legal Guardian or Personal Representative of an Adult

Request for proxy access as legal representative of an adult

Form - Legal Representative Requesting Proxy Access of an Adult

  • You are the legal guardian or personal representative of an adult patient and have legal authority to make health care decisions on behalf of this patient
  • You must include documentation proving you have legal guardianship or other legal authority such as a Durable Power of Attorney for Health Care
  • If the relationship changes and you no longer have a legal right to make health care decisions on behalf of the patient, you will inform Bryan Health and end all proxy access to the patient's information

Adult Granting Access to Another Adult

Granting access to your MyChart account to another adult

Form - Adult Granting Access to Another Adult

  • You are granting access to another adult to view your MyChart account
  • You can choose the level of access: full access, scheduling and messaging only, or chart view only
  • This could include access to information available as part of your health record or messages you send through your MyChart account
  • Some of this information could be very personal and sensitive, such as test results, treatments and medical opinions
