Access Family Health Records

Request access to view health records of others in MyChart

With MyChart, you can request or grant access to view records of your children, spouse, parent or another adult. This is called proxy access.

You can request or grant access as a:

- You can also drop the form off at your doctor's office or the Health Information Management office at Bryan Health hospitals
You can expect access within 24-48 business hours after the time we receive your form and any required documentation
- If you have a MyChart account, you will see this person in a new 'circle' at the top of your MyChart account on your computer or tablet; if you are using the MyChart App, the 'circle' will be at the bottom of the screen
- If you don't have a MyChart account, you will receive an email with information on how to open/activate your account
  - If you did not provide an email address, you will receive a letter with the information you need to set up your MyChart account
- If you do not receive a response with 48 business hours, please call 402-481-8066 or email mychart@bryanhealth.org

You will be able to view records, test results and other information, and perform all functions (i.e., schedule appointments) just as a patient can in MyChart, with the exception of protected tests, per Nebraska law
Information available as a proxy will become limited upon the child's 13th birthday and access will terminate automatically on the child's 19th birthday

You are the legal guardian or personal representative of an adult patient and have legal authority to make health care decisions on behalf of this patient
You must include documentation proving you have legal guardianship or other legal authority such as a Durable Power of Attorney for Health Care
If the relationship changes and you no longer have a legal right to make health care decisions on behalf of the patient, you will inform Bryan Health and end all proxy access to the patient's information

You are granting access to another adult to view your MyChart account
You can choose the level of access: full access, scheduling and messaging only, or chart view only
This could include access to information available as part of your health record or messages you send through your MyChart account
Some of this information could be very personal and sensitive, such as test results, treatments and medical opinions
