We are pleased to offer the Crete Area Medical Center Patient Portal for your use. The Patient Portal is available over the internet, which means that you can use it from virtually anywhere. We hope this tool will help you take an active role in your health care.

What do you need to enroll?
Your Email address
Crete Area Medical Center will need to record your email address and associate it with your medical record number. You will be asked for this information at admissions during your clinic or hospital visit.
Your Medical Record Number
Your will need to know your medical record number, which is found on the printed header at the top of the discharge information packet provided after your visit or on your bill. This medical record number will also be provided at admissions if you indicate you would like to be a portal user.
