This program is designed to pick up where your provider left off, equipping you with the tools to make habit change possible!
What is Health Coach Included?
Health Coach Included are two 30-minute open-coaching sessions that focus on you, your health and permanent live change while offering accountability, encouragement and lots of new perspectives.
How much does the program cost?
This new service is FREE to our Crete Area Medical Center patients and employees because you are one of us, and we want you to be successful.
How to Get Started
Health Coach Included may happen through referral. Referrals can be made in two easy ways:
Want More Coaching?
Additional sessions are available for purchase:
- $100 for 8 sessions (25 minutes each)
- Use them anytime over the following six months
- Available by phone or in person
