2026 Call for Posters

For Bryan Health Connect Annual Conference in partnership with Heartland Health Alliance April 28-29, 2026

Invite BHC and HHA members to participate in the annual meeting by submitting posters that showcase contributions to improving healthcare and lowering costs. Posters will be reviewed for innovation, impact, and measurable results, with awards presented during the conference dinner.

• Submission deadline: March 2
• Final poster slide due: April 22
• Presentation date: April 28 (includes Q&A time)

• Poster Template – A blank PowerPoint template for creating your poster. Includes placeholders for:
  • Project Title
  • Aim statement
  • Lessons learned
  • Run charts/data
  • Why the project is important
  • Recommendations and next steps
  • Team members
• Readmissions Example – Completed example focused on reducing 30-day all-cause readmissions. Shows layout, narrative style, and how to highlight measurable results and next steps.
• A1C Reduction Example – Example of a completed poster focused on reducing A1C with team-based care. Shows how to structure content and visuals.
