The patient portal is completely secure, so you can be confident that your private information is protected. Only you – or an authorized family member – can access your patient portal.

It’s easy to sign-up for your patient portal account. You just need:

  • Your Email Address – we will associate your email address with your records, you just need to make sure that CAMC has your personal email address
  • Your Medical Record Number – you can find this in the printed header on the top page of your discharge information or on your billing statements

What is the patient portal & what benefits does it offer?

The patient portal is a secure, online way for you to view your patient chart any time you want.

Through your patient portal you can:

  • Email your providers in a secure way
  • View, download or transmit your health summary.
  • Request appointments and see details for upcoming visits
  • See laboratory results and radiology reports
  • View your allergies and conditions
  • Track your immunizations and other health information
  • Review your medications
  • See your visit history, including discharge instructions from CAMC visits

How do I reset my password or change my temporary password?

To reset your password, go to the patient portal log-in screen. Click the 'Forgot Password?' link. Fill in your Patient Portal Log-on ID and the email address you provided Crete Area Medical Center and click 'Submit'. You will receive a message in the email account you designated with instructions to reset your password.

How do I find my Log-on ID, or what if I don’t know it?

How do I find information in my patient portal?

How do I view my medical chart information?

On the left sidebar menu, click on the available chart item you want to view.

How do I view or send messages?

After you log-in to your patient portal, click the ‘Messages’ button. You can then view and send messages.

How to I get back to the patient portal home screen?

Use the 'Home' button in the upper right corner of the screen. DO NOT use the back button in your browser.

How do I request an appointment using my patient portal?

After you log-in to your patient portal, click the ‘Appointments’ button. Then click the ‘Request a New Appointment’ button on the right side of the screen. Select your appointment type and reason, then click continue. Choose your preferred appointment times or select no preference, then click continue. Provide your contact information and a scheduling staff member will contact you directly to confirm an appointment time.

You can always call us at 402-826-2102 for assistance in scheduling an appointment as well.

Can I request a medication refill using my patient portal?

No. We are not able to accept refill requests through the patient portal at this time. Please contact your pharmacy to request refills of your medications.

What if I see information that needs to be updated in my chart?

If you see a discrepancy or have an update to provide our clinic, you can send a message to the clinic staff through your patient portal, or you can call 402-826-07926.

Will I be able to see my child's patient chart?

Due to potential HIPAA privacy concerns, we are not able to grant access to a child's chart at this time.
