Medicare Certified Skilled Care
Our medical home team of physicians, nurses and rehabilitation specialists are pleased to provide you the highest level of Medicare Certified Skilled Care. Our goal is to restore you back to your highest level of function and get you back home and into a daily routine. We will work with you from the day of admission to establish goals and develop plans for your return home.
What is Medicare Certified Skilled Care?
Medicare Certified Skilled Care, often referred to as Swing Bed, is a level of care that requires the direction of a physician and 24-hour management by a nurse and other skilled personnel. It is designed to be a short-term stay option that aids in your rehabilitation.
How is Skilled Care different?
While in Skilled Care, your physician oversees your care and only makes weekly visits to round on you. Your daily care is managed by your nurse and rehabilitation teams.
Who qualifies for Skilled Care?
Patients must meet guidelines established by Medicare/Medicaid to qualify for Skilled Care. You will have a Care Plan Team that meets weekly to discuss your progress. You and your family are encouraged to attend these meetings.
Who is on my Care Plan team?
This is a multi-disciplinary team. Representatives from the following areas might be included on your team: physical therapy, occupational therapy, dietary, social services, physicians and nurses.
What if I no longer qualify for Skilled Care?
Once you have progressed on your Care Plan goals and/or no longer meet Skilled Care criteria, we work with you to determine plans for going home or transferring to another facility.
