What do you need to enroll?
Your Email Address
Crete Area Medical Center will need to record your email address, so you can associate that email address with your records. You will be asked for this information when registering.
Your Medical Record Number
You will need to know your medical record number. It is found in the printed header on the top of the page of discharge information or billing statements you receive.
Using the Patient Portal
In order to use the medical portal, you will need to verify that Crete Area Medical Center has your current personal email address. You will also need the medical record number found on your bill or discharge information.
