BryanLGH's Crete Area Medical Center receives highest national Medical Home Recognition
BryanLGH's Crete Area Medical Center has been recognized as a national leader in the successful implementation of a new healthcare approach that promotes high quality, cost-effective patient care.
This model of care, called Patient-Centered Medical Home, fosters collaboration among primary care physicians as well as specialty physicians to promote an integrated approach to care. Such practices have are shown to improve patient outcomes and are a key strategy in the nation's healthcare reform efforts.
The Physician Clinic at Crete Area Medical Center received Level 3 Patient-Centered Medical Home Recognition from the National Committee for Quality Assurance (NCQA) Physician Practice Connections. This is the highest achievable Patient-Centered Medical Home Recognition for a physician practice. Crete Area Medical Center is the first rural health facility in Nebraska to earn Level 3 Recognition, and only the fourth physician's practice statewide.
"Our Medical Home program has improved patients' access to health care, communication with patients and quality of care our patients receive," stated Dr. Russell Ebke, Chief of Medical Staff at Crete Area Medical Center. "We utilize our electronic medical records to track test results, follow patient referrals, identify patients in need of follow-up care and reach out to patients with preventative services," stated Ebke.
NCQA's Patient-Centered Medical Home program recognizes physician practices that prioritize the strengthening of the physician-patient relationship, coordinate care for patients across multiple settings, and engage in a team approach to improving patient care.
"Research shows that identifying and regularly seeing a primary care provider will lead to lower healthcare costs and better outcomes for patients," stated Carol Friesen, BryanLGH Vice President and Crete Area Medical Center CEO.
"Becoming designated as a Level 3 Medical Home is an outstanding achievement for Crete Area Medical Center and for the community of Crete. It reflects a commitment to clinical quality with an emphasis on continuity of care, electronic medical records and overall community wellness," stated Collyn Florendo, president of the Crete Area Medical Center Board of Directors. "It confirms the quality of care we offer our community."
BryanLGH's Crete Area Medical Center is an integrated healthcare facility that includes an in-house Physician's Clinic, Critical Access Hospital and Emergency Department supported by an in-house laboratory, pharmacy, surgery, radiology and rehabilitation department.
